Who we are
This website is for the office of Dr. McKay in Portland, Oregon. Our website address is https://drdebmckay.com. We may change this privacy policy from time to time by updating this page. You should check this page from time to time to ensure that you agree with any changes. The last update of this Privacy Policy was made on May 23, 2018.
Contact forms
Contact form submission data — IP address, name, email address, and message — is submitted to the Akismet service owned by Automattic for the sole purpose of spam checking. The actual submission data is not stored in the database of the site but is emailed via SSL directly to the office of Dr. McKay. This email will include a timestamp, name, email address and message. This information may be printed for our files. Only the information you fill out on the form is sent, and we do not collect information such as your IP address when you contact us. None of the information is stored on our website.
Embedded content from other websites
Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website.
These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracking your interaction with the embedded content if you have an account and are logged in to that website. We do not track this information and will always include information about the source of this content so you are able to check their websites for their data policy.
Analytics
WordPress.com Stats
Data Used: IP address, WordPress.com user ID (if logged in), WordPress.com username (if logged in), user agent, visiting URL, referring URL, timestamp of event, browser language, country code. Important: The site owner does not have access to any of this information via this feature. For example, a site owner can see that a specific post has 285 views, but he/she cannot see which specific users/accounts viewed that post. Stats logs — containing visitor IP addresses and WordPress.com usernames (if available) — are retained by Automattic for 28 days and are used for the sole purpose of powering this feature.
Activity Tracked: Post and page views, outbound link clicks, referring URLs and search engine terms, and country. When this module is enabled, Jetpack also tracks performance on each page load that includes the Javascript file used for tracking stats. This is exclusively for aggregate performance tracking across Jetpack sites in order to make sure that our plugin and code is not causing performance issues. This includes the tracking of page load times and resource loading duration (image files, Javascript files, CSS files, etc.).
Who we share your data with
We do not share any of our information with anyone. All interactions with other sites such as our prescription service and appointment booking fall under their data policy. We use two vendors to make it easy for you to book appointments and order prescription refills.
- Prescriptions – We use the Fullscript service. Their privacy policy is located here.
- Online appointment booking – Jane. Their privacy policy is located here.
Data retention
We do not retain any of your data online. However, any information we receive via the contact form will be printed/scanned and retained in your file if you become a patient. If you do not become a patient, this information will be deleted from our files in 90 days. As we do not retain any information online, we do not offer a function for you to export or erase personal data.
